Support that follows the person

Support that follows the person

What I heard

Participants wanted services that are easy to find, respectful and connected across organizations. They described active outreach and practical help that continues instead of requiring a person in crisis to navigate every program alone.

Why it matters

People seeking help may be dealing with housing loss, illness, disability, trauma or limited access to transportation and technology. Repeating the same story to disconnected services creates another barrier to receiving support.

What I learned...

Participants valued continuity and navigation as much as the individual programs themselves. A service can exist on paper and still remain inaccessible if no one helps residents reach it.
Jurisdiction notes
Peterborough Social Services can connect residents with Ontario Works, housing, food, child care and community referrals. A truly coordinated system also depends on provincial health and income programs, community agencies and shared information and referral practices.
